What the GSA Expects in an AI Incident Log

When the GSA released the draft for the new AI safeguarding clause, GSAR 552.239-7001, the 72-hour reporting window became a primary focus for many people in the federal contracting space. Three days is a tight turnaround, especially when you are dealing with something as complex as an AI performance drift or a suspected security breach. A federal AI incident log is far more detailed than a standard IT ticket, and everything it needs might not seem self-explanatory. It requires a specific set of technical forensics and narrative data to satisfy the new transparency requirements.

Defining a "Performance Incident"

One of the biggest shifts in this new framework is that "incidents" are not limited to traditional hacks or data leaks. Performance issues now fall under the same reporting umbrella. If your model begins to exhibit significant hallucinations, shows a sudden bias drift, or starts providing unauthorized routing decisions, the clock starts ticking the moment you suspect the issue.

For a federal agency, a model that provides an incorrect policy summary is just as much of a liability as a server being down. This means your internal logs need to be tracking more than just uptime. You need a record of how the model is behaving in the field.

The Components of a Professional AI Log

To meet the GSA standard, a well-designed incident log needs to capture several distinct layers of information.

  • Discovery and Classification: You need a clear reference number, the exact timestamp of discovery, and a classification. Is this a security breach, a performance failure, or a bias-related event? 

  • Narrative Description: This should be a dry, objective account of what happened. Stick to the facts and avoid speculation about why it happened until the investigation is further along.

  • Technical Traceability: This is the most demanding section. The government now expects to see summarized intermediate processing actions. If you are using Retrieval-Augmented Generation, you must be able to attribute exactly which sources were pulled and how the model routed its final response. 

  • Immediate Mitigation: What did the team do in the first hour? Whether you rolled back to a previous model version or temporarily disabled an API, every corrective action must be time-stamped.

Effective reporting depends on a clear line of communication between your technical team and your Contracting Officer. It is easy to get buried in the raw data, but the person on the other end of that report needs a clear path to understanding the actual risk. Designating a specific point of contact who can translate technical logs into a concise summary ensures that your 72-hour response is both fast and coherent. This person acts as the bridge, making sure that the complex details of a model drift or a security event are presented in a way that aligns with the broader mission goals.

Maintaining the 90-Day Artifact Trail

Reporting the incident is only the beginning. Under the latest guidelines, contractors are required to provide daily status updates until the issue is fully resolved. Beyond that, you have to preserve the evidence. All logs, forensic images, and incident artifacts must be kept for a minimum of 90 days. 

For a small company, managing this level of documentation can be a heavy lift. However, having a standardized template ready to go before an incident occurs is a massive advantage. It prevents the last-minute scramble and ensures you are providing the government with the high-quality, transparent data they now require.

Building a culture of documentation is about more than just compliance. It is about proving that your firm has a grip on its technology. When you can hand over a detailed, professional log within that 72-hour window, you are demonstrating a level of maturity that federal agencies value in their partners.
